I am the Parent / Guardian of this child and agree to the follow terms & conditions.

As parent/guardian of the above child, I hereby authorize his/her participation in the Eric Thomas All Sports Skills Camp. I agree to release Eric Thomas, Champion Trainers, the Eric Thomas Foundation and all employees from responsibility for injuries incurred during the camp and/or any travel to or from camp. Should injury occur, I authorize employees of Champion Trainers to arrange for any medical attention which they deem necessary. Specific risks vary from one exercise to another and risks range from minor injuries to major injuries. In consideration of your participation in the activities offered by Champion Trainers, you understand and voluntarily accept this risk and agree that Champion Trainers will not be liable for any injury, including, without limitation, personal, bodily, or mental injury, guests, unborn child, or relatives resulting in negligence of Champion Trainers or anyone on Champion Trainers' behalf using the facilities, whether related or not. You acknowledge and agree that you have read the foregoing and know of the nature of the activities with Champion Trainers and you agree to all terms of this agreement.
